Each item in the boxes was rated as very good (3 points), adequate (2 points), doubtful (1 point) or inadequate (0 points). Convergent validity was assessed with Box 9.a (score/12), discriminative validity with Box 9.b (score/9), reliability with Box 6 (score/9) and responsiveness with Box 10.a (score/9)
